ANDERSEN AIR FORCE BASE, Guam -- The 36th Medical Group is among a growing number of health care facilities that is committed to planning, designing and delivering patient and family-centered care. Our goal is to listen to our patients and their families, our customers, and include them as partners in helping us provide the highest quality of care.

We know that our patients and families give a valuable perspective into how we can deliver better patient and family-centered care and we welcome the opportunity to build that partnership. To succeed on this journey, we invite you to join us, as vital partners, to be a member of the Patient and Family Advisory Council.

The Patient and Family Advisory Council, or PFAC, is a new initiative at the 36 MDG this year. The intent of this council is to advise the 36 MDG leadership team as we strive to become one of the best clinics in the Military Health System and our community. We care about what our customers think and want to incorporate their feedback and ideas into our processes to provide safe, quality, trusted care.

If you want to get involved in this exciting initiative and become a PFAC member, please complete the PFAC application and submit it to Staff Sgt. Alana Pool at alana.pool@us.af.mil.

Thank you for your interest in joining us! We are excited that you want to partner with us on our journey to improve our delivery of patient and family-centered care!
