Department of Agriculture Requirements

Import Permit – It is vital to understand that bringing a pet to Guam without a valid Entry Permit can result in imprisonment of the pet owner and a fine not to exceed $1,000.

** It is important to always check with the Guam Department of Agriculture to ensure you have the most recent entry requirements as they can change without notice.  The recommendations listed below are current as of 2 September 2021**

Programs:

  1.  120 day Quarantine: Pets arriving from non-exempt areas without a passing FAVN or other requirements will be required to quarantine at an approved quarantine facility at the owners cost.
  2. Calculated Quarantine: Pets arriving from non-exempt areas with a FAVN of 0.5 IU to 0.99 IU will spend only as many days in commercial quarantine as required to reach 120 total days since the FAVN blood sample was received by the laboratory.
  3. 5-Days or Less with Home Quarantine: Pets originating from US Mainland or military personnel located on military installations with a FAVN of 1.0IU or greater and all other vaccinations will be examined by a veterinarian within 5 days of arrival and will complete the remainder of their 120 day quarantine at home.
  4. Exempt: Pets that have been in an exempt country or area for at least 120 days will not be required to quarantine and may be released directly from the Airport.  Owners must contact Guam Customs and Quarantine to schedule clearance at 671-642-8071/2.
    1. Exempt areas: British Isles, Australia, New Zealand, Japan, and Hawaii on direct flights to Guam.

Requirements:

  1.  Application for import permit – submit completed application with a non-refundable fee of $65.00 per pet to Guam Department of Agriculture.  Documents (rabies certificates, vaccination records, FAVN, Health Certificate) can be emailed to quarantine@doag.guam.gov and payment can be sent via mail or authorized representative.  Only money orders or certified checks may be submitted and should be made payable to “Treasurer of Guam”.

  1. Affidavit for export – This only applies to those individuals coming from exempt areas. Issued by a government entity (state national department of agriculture veterinarian or U.S. Army veterinarian) certifying the dog or cat was not outside of the area of origin during the 4-month period immediately preceding the date of shipment.
  2. Vaccinations –
    1. Rabies –
      1. Two vaccines in lifetime of animal
      2. 1 or 3 year vaccine accepted
      3. Most recent vaccine must not expire prior to release from quarantine.
      4. Subsequent rabies vaccines must not be administered less than 30 days following the first rabies vaccine.
      5. Rabies certificate must include manufacturer, serial/lot number, expiration date, printed full first and last name, clinic address, license number and signature of veterinarian who administered the vaccine.
    2. Other Required Vaccines – must not be administered less than 10 days before departure and must still be current on arrival in Guam.

        1. Canine Distemper Virus
        2. Canine Infectious Hepatitis Virus or Canine Adenovirus-2
        3. Canine Leptospirosis
        4. Canine Parinfluenza
        5. Canine Parvovirus
        6. Canine Bordatella bronchiseptica (Kennel Cough)
      2. Cats
        1. Fenline Panleukopenia Virus
        2. Feline Viral Rhinotracheitis (herpesvirus-1)
        3. Feline Calicivirus

    1. Heartworm test
    2. Fecal Parasite test
    3. External Parasite check by an accredited veterinarian
  4. Electronic Microchip – must be US made and readable with an AVID universal scanner

  1. Test results must include: microchip number and complete description of the animal (sex, breed, age, color)
  2. RABIES VACCINES CANNOT BE ADMINISTERED AFTER BLOOD IS DRAWN FOR THE FAVN OR THE FAVN WILL BE INVALID REGARDLESS IF IT IS PASSING OR NOT.   
  3. To qualify for 5-day or less program, FAVN must be greater than 1.0 IU/ml

    2. If internal or external parasites were observed, pets must be treated for both by an accredited veterinarian within 14 days of travel and these products (ingredients, manufacturer) and treatment dates must be listed on the health certificate.

Your pet will arrive at Won Pat International Airport (All Commercial Travel) or the Andersen Passenger Terminal (Patriot Express).  Pets will be received by Andersen Pet Lodge staff member and be transported directly to the quarantine facility.

Your pet will be examined by an Army veterinarian within 5 days of arrival to the quarantine facility.  The release is $15/pet plus a $2 user fee per transaction (i.e. 1 pet = $17, 2 pet = $32).  You must pay this fee directly to the Andersen Veterinary Clinic prior to your pet being released from the quarantine facility.  It is advised to pay this ahead of arrival to streamline release processes.  The quarantine release does not suffice for establishing a valid doctor patient relationship.  A separate wellness or sick call examination must be scheduled for any healthcare needs.

While under the care of the Andersen Pet Lodge your pet will receive meals and a clean kennel.  Pet lodge personnel may or may not be able to medicate your pet for various reasons.  It is imperative you discuss any medications your pet requires at time of booking.  No medical services will be provided unless staff are concerned about life, limb or eyesight.  Any medical services provided will be required to be paid by the owner.  If your pet requires vaccinations or tests due to improper preparation for arrival to Guam you as the owner will be required to pay for the veterinarian examination, any vaccinations administered, tests provided and shipping of tests if applicable.

Pet Release: Picture identification is required for release from quarantine.  You must bring a suitable leash or crate for transport.  ALL FEES MUST BE PAID PRIOR TO RELEASE.
